Billing/Insurance
General Information
We participate with most major insurance carriers, including Blue Cross/Blue Shield, area HMOs and PPOs, Champus, and Medicare. Our Patient Accounts Department will gladly file your medical claims for you. Please keep in mind that your medical insurance is a contract between you and your insurance carrier. We will provide them with the appropriate information they need to pay for your medical care. However, you are responsible for the payment of any deductibles, co-payments, or remaining balances at the time of your visit. If your insurance carrier does not settle your account in full within 90 days, you will be responsible for payment of services. Please also note that there is a $25.00 fee for any returned checks in addition to any fees your financial institution may charge.
To help facilitate the filing of your claim and to keep your patient records up to date, please notify our office of any changes to your name, address, telephone number and/or insurance carrier.
